About the role

Location: 611 W County Line Rd S, Fort Wayne, IN 46814. Position: Licensed Practical Nurse. Shift: 7:00 am - 7:00 pm, 7:00 pm - 7:00 am. Job Type: Full Time on Night Shift and Part Time on Day shift.

Responsibilities

• Lead, manage, and train your team of caregivers.
• Responsible for evaluating up to 133 residents and reporting change in condition to physicians.
• Utilizing Electronic Health Record (EHR) and Electronic Medication Administration Record (EMAR)
• Communicating effectively to team, residents and families.
• Building strong relationships.

Requirements

• Licensed Practical Nurse (LPN) license in the state.
• Assisted Living/Memory Care experience preferred.
• Passionate about working with the elderly.

Pay

$33 per hour (base pay). $2 shift differential for working night shift.
