Licensed Practical Nurse - Case Coordinator

About the role

Responsible for utilizing medical knowledge to assess patient needs for the purpose of developing a plan for assisting patients to achieve their optimal level of health. Duties include telephonic assessment, care planning, implementation, coordination, monitoring, evaluation, and outcomes measurement within the scope of their license. The LPN care coordinator is a patient advocate that promotes safety and maintains privacy and confidentiality in accordance with policies and procedures.

This position is full-time, Monday - Friday, with no weekends or nights. Initially an in-office position, it transitions to a hybrid option (two days working from home and three days in the office) after successful completion of the orientation period.

Responsibilities

• Telephonic assessment of patient needs
• Care planning, implementation, and coordination
• Monitoring, evaluation, and outcomes measurement
• Patient advocacy promoting safety and privacy
• Rare travel for onsite evaluations at homes, hospitals, etc.

Requirements

• LPN with an unrestricted license/certification in the state or in a state with licensure reciprocation
• Successful completion of case management/disease management orientation program
• Minimum of 2 years full-time equivalent of direct clinical care to the consumer
• Excellent communication skills
• Basic typing/computer knowledge with a minimum keyboarding speed of 35 WPM
• Previous case management experience helpful but not required

Pay

$23.00–$25.75 hourly
