Licensed Practical Nurse Cardology

About the role

Supports the AdventHealth mission by extending the healing ministry of Christ.

Responsibilities

• Evaluates reports to identify patients in need of preventive care and documents, monitors, and reports on these efforts.
• Makes sure Medicare patients receive their Annual Wellness Visits and sets up a documented process to ensure scheduling.
• Engages in regular huddles and uses communication tools to meet patient care needs.
• Participates in planning, implementing, and collecting data for quality improvement initiatives.
• Collaborates with physicians, office staff, and management to report data and perform analytics reflecting quality metrics.
• Ensures care gaps are met through motivational interviewing, standard orders, and protocols, and educates patients.
• Maintains care measures according to standards and documents and reports appropriately.
• Runs reports to identify patients who haven't had appointments for up to two years and activates processes to schedule them.

Requirements

• Associate's Degree or Graduate of a Technical School in relevant field.
• 1+ position-related experience; 2+ preferred.
• Licensed Practical Nurse (LPN).

Pay

$20.95 - $38.96 per hour.

Schedule

Full-time, Monday to Friday, 8:00 AM to 4:30 PM.
