About the role
Provides direct patient care in a skilled nursing facility setting. Assists patients with activities of daily living, administers medications, and monitors patient health status.
Responsibilities
- Administer medications as prescribed by physicians
- Assist patients with bathing, dressing, eating, and other activities of daily living
- Maintain accurate patient records and documentation
- Monitor patient health status and report changes to healthcare providers
- Participate in shift handovers and ongoing team meetings
Requirements
- Current Virginia LPN license
- At least one year of recent relevant experience
- Bilingual in English and Spanish preferred
Qualifications
- High school diploma or equivalent
- Successful completion of an accredited LPN program
- Basic computer skills
Skills
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of medical terminology and procedures
