About the role

The Licensed Practical Nurse is responsible for providing direct patient care, ongoing education and reinforcement of care plans, and providing general support in an ambulatory setting under the direction and supervision of the patient’s physician or licensed medical practitioner or designee in accordance with policy and procedure and competency to promote patient health and wellness.

Responsibilities

  • Provides services to all age populations in a manner that demonstrates an understanding of the functional/developmental age of the individuals served.
  • Records and relays patient information, symptoms, and concerns to the patient’s medical provider or registered nurse for further triage assessment and to determine a plan of care.
  • Aid patients with therapeutic regimens.
  • Reinforces established plans of care and age-appropriate interventions.
  • Reinforces patient/family teaching plan based on identified health education needs and the condition and age of the patient through approved educational materials or written instructions.
  • Administers tasks, treatments, and medications in accordance with validated competencies.

Qualifications

  • Education: High School Diploma required. Graduate of accredited Practical Nursing Program required. Completion of a nursing program as approved by the State Board of Nursing.
  • Experience: Minimum of 1 year of nursing experience preferred.

Additional Skills/Requirements

  • Licensure: Licensed to practice as a Practical Nurse (LPN) in the Commonwealth of Virginia prior to onboarding.
  • AHA Healthcare Provider BLS required.
  • Ability to successfully complete general and department role based specific skills validation and competency testing.

  • Communication: Communicates effectively and professionally with all members of the interdisciplinary team, patients, medical providers and all levels of healthcare. Conveys information accurately to physicians, nurses, and other team members, as well as listen and respond to the needs and concerns of patients and colleagues. Utilizes appropriate communication tools when communicating within the interdisciplinary team. Documents patient information in EHR.
  • Teamwork: Willing to share responsibilities, support their teammates, and work together to achieve common goals. Organizes and prioritizes daily work assignments to support patient needs, the interdisciplinary team, and operational efficiency. Demonstrates teamwork in the delivery of patient care. Recognizes the importance of time sensitive issues.

Physical Demands

The job requires constant standing, frequently walking, and bending/stooping. Proficient communicative, auditory, and visual skills. Attention to detail and ability to write legibly. Ability to lift/push/pull 100 lbs. For any weight over 35 lbs., use Safe Handling Equipment. Pay

The base compensation range for this role is $23.00 - $34.37 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education. Please note that the pay range does not include any applicable incentive compensation programs such as shift differentials, clinical ladders, and other incentives that may be available for those in eligible positions.
