Responsibilities
- One to One patient care in home/school environment
- Implement and document the plan of care to ensure quality and continuity of care for patient
- Administration of prescribed medication, treatments and therapies
- Patient assessments; coordination of care
- Reports any changes in the client condition to Clinical Manager and physician
- Health promotion, teaching, and training of family members
Requirements
- Current Pennsylvania State Licensure as a Licensed Practical Nurse (LPN)
- Valid Driver’s License
- Passing score on the NCLEX-RN
- Experience in home health care preferred

Schedule
Immediate Need: Thursday - Sunday 12hr shifts 6:30a - 6:30p.
Pay
$28-$32 per hour
