Responsibilities
- One to one patient care in home/school environment
- Implement and document the plan of care to ensure quality and continuity of care for patient
- Administration of prescribed medication, treatments and therapies
- Patient assessments; coordination of care
- Reports any changes in the client condition to Clinical Manager and physician
- Health promotion, teaching, and training of family members
Requirements
- Current Pennsylvania State Licensure as a Licensed Practical Nurse (LPN)
- Valid Driver’s License
- Passing score on the NCLEX-RN
- Experience preferred but not required

Schedule
- Sunbury, PA: Thursday and Friday 6:30pm to 6:30am, Saturday 8:30am to 8:30pm and Sunday 10:30pm to 6:30am
- Selinsgrove, PA: 8am to 4pm 5 days a week
- Northumberland, PA: 7am to 3pm 7 days a week or Monday through Friday
Pay
- $28-$32 per hour
