Responsibilities
- One to One patient care in home/school environment
- Implement and document the plan of care to ensure quality and continuity of care for patient
- Administration of prescribed medication, treatments and therapies
- Patient assessments; coordination of care
- Reports any changes in the client condition to Clinical Manager and physician
- Health promotion, teaching, and training of family members

Pay
- Competitive pay + weekly pay with direct deposit
Schedule
- Cresco, PA - Friday and Saturday 11pm to 7am (times can be flexed if needed) and Saturday and Sundays 7am to 3pm (times also flexible if needed)
- Tobyhanna, Pa - LPN for g-tube care , PART-TIME
- Tobyhanna, PA - LPN for trach/vent care , PART-TIME
- Lake Ariel, Pa - LPN Weekends Saturday and Sunday 7am-11pm Shifts can be split has 18 hours a day on both days / Monday , Thursday and Fridays 6pm-6am
