Licensed Practical Nurse

About the Role

The Licensed Practical Nurse provides direct nursing care to residents in accordance with current federal, state, and local standards, guidelines, and regulations governing the community. This role ensures the highest degree of quality care is maintained at all times under the direction of the RCC Services or Nurse Supervisor.

Responsibilities

• Comply with the community’s personnel, safety, and corporate policies and procedures, as well as nursing department policies.
• Report allegations of resident abuse or neglect immediately to the Executive Director, RCC, or Unit Manager.
• Treat residents, family members, visitors, and team members with dignity and respect while maintaining confidentiality.
• Assist in the admission, transfer, and discharge of residents.
• Provide daily hygiene care, including bathing, oral care, hair and nail care, and assistance with bowel and bladder functions.
• Keep residents dry by changing clothing and linen when soiled or wet.
• Assist in transporting residents to appointments, activities, and social programs.
• Lift, turn, move, position, and transport residents using proper techniques and equipment (e.g., lifts, wheelchairs).
• Report and document changes in a resident’s condition or behavior to the Charge Nurse.
• Provide care for dying residents and perform post-mortem care as needed.
• Administer professional services such as catheterization, tube feedings, suction, wound care, colostomy care, and blood draws as required.
• Perform lab tests, blood pressure monitoring, and other routine tasks per resident care plans.
• Complete accident/incident reports, medical forms, evaluations, and charting in an informative and descriptive manner.
• Ensure accurate recordkeeping for admissions, transfers, and discharges, including nurses’ notes and care documentation.
• Report discrepancies in physician’s orders, diet changes, or charting errors to the Nurse Supervisor.
• Develop and maintain good working relationships with inter-departmental personnel to support resident needs.
• Review complaints and grievances from assigned personnel and report them to the Nurse Supervisor.
• Receive and give nursing reports at the start and end of shifts.
• Report occupational exposures to blood, body fluids, infectious materials, or hazardous chemicals per community policies.
• Monitor assigned personnel to ensure compliance with safety regulations for equipment and supplies.
• Recommend equipment and supply needs to the Nurse Supervisor.
• Review care plans daily to ensure appropriate care is provided.
• Oversee resident needs, staff, and operations as needed.

Requirements

• Valid professional LPN license in good standing with the state.
• Basic computer skills; ability to read, write, speak, and understand English.
• Ability to work beyond normal hours, including weekends and holidays, as necessary.
• Ability to assist in resident evacuations during emergencies.
• Capability to perform essential job functions with or without reasonable accommodation.

Physical Requirements

• Regular attendance at the worksite.
• Frequent lifting (up to 25 lbs), occasional lifting (26–50 lbs), and constant carrying (up to 10 lbs).
• Constant reaching at or below shoulder height; frequent reaching above shoulder height.
• Constant pushing/pulling and hand manipulation (grasping, handling, fingering).
• Frequent twisting, bending, crawling, squatting, kneeling, crouching, climbing, and balancing.
• Ability to operate an automobile and assist with various physical activities (e.g., games).
• Ability to work with ill, disabled, elderly, emotionally upset, or hostile individuals.
• Sufficient vision and hearing (or use of prosthetics) to perform job requirements.
