Must have a current, unrestricted LPN/LVN license in the state of practice. Must have a minimum of one (1) year of experience in a long-term care, geriatric nursing, or sub-acute care setting. Must have CPR certification. Must have excellent communication and interpersonal skills. Must be able to work effectively as part of a team. Must be able to demonstrate compassion, empathy, and patience in working with residents and their families. Must be able to work in a fast-paced environment and handle multiple priorities. Must be able to maintain confidentiality and adhere to HIPAA regulations. Preferred Qualifications Experience with electronic health records (EHR) systems. Bilingual (English/Spanish) is a plus. About the role

Provide direct nursing care to residents in accordance with current federal, state, and local standards, guidelines, and regulations governing the Community. Work under the direction of the RCC Services or Nurse Supervisor to maintain the highest degree of quality care at all times.

Responsibilities

  • Comply with the Community’s personnel, safety, and corporate policies and procedures, as well as nursing department policies.
  • Report witnessed or reported allegations of resident abuse or neglect immediately to the Executive Director, RCC, or Unit Manager.
  • Treat residents, family members, visitors, and team members with dignity and respect; maintain confidentiality of all resident and family information.
  • Assist in the admission, transfer, and discharge of residents.
  • Provide daily hygiene care including bathing, oral care, hair and nail care, and assist residents with bowel and bladder functions; collect specimens when required.
  • Keep residents dry by changing clothing and linen when soiled or wet.
  • Assist in transporting residents to and from appointments, activities, and social programs.
  • Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, and lifts.
  • Report and document any changes observed in a resident’s condition or behavior to the Charge Nurse.
  • Assist in the care of dying residents and provide post-mortem care.
  • Administer professional services such as catheterization, tube feedings, suction, applying and changing dressings, colostomy care, and taking blood as required.
  • Perform lab tests, blood pressures, and other routine monitoring as required per the resident care plan.
  • Complete accident and incident reports as necessary.
  • Perform administrative duties including medical forms, reports, evaluations, charting, and recordkeeping upon resident admission, transfer, or discharge.
  • Chart nurses’ notes in an informative and descriptive manner reflecting care provided and resident response.
  • Report discrepancies concerning physician’s orders, diet changes, or charting errors to the Nurse Supervisor.
  • Develop and maintain good working rapport with inter-departmental personnel to ensure nursing services meet resident needs.
  • Receive and give nursing reports at the start and end of shift duty hours.
  • Report occupational exposures to blood, body fluids, infectious materials, or hazardous chemicals according to Community policies.
  • Monitor assigned personnel to ensure compliance with safety regulations in the use of equipment and supplies.
  • Recommend equipment and supply needs of your unit or shift to the Nurse Supervisor.
  • Review care plans daily to ensure appropriate care is being rendered.
  • Oversee resident needs, staff, and operations as needed.

Requirements

  • Valid, unrestricted LPN license in good standing with the State.
  • Minimum of one year of experience in long-term care, geriatric nursing, or sub-acute care.
  • CPR certification.
  • Basic computer skills; ability to read, write, speak, and understand English.
  • Ability to work beyond normal working hours, weekends, and holidays when necessary.
  • Ability to assist in the evacuation of residents during emergency situations.
  • Ability to perform essential position functions with or without reasonable accommodation.

Physical requirements

  • Lift up to 10 lbs constantly, 11 to 25 lbs frequently, 26 to 50 lbs occasionally, and over 50 lbs occasionally.
  • Carry up to 10 lbs constantly, 11 to 25 lbs occasionally, 26 to 50 lbs rarely, and over 50 lbs not required.

  • See and hear adequately (with or without prosthetics) to meet position requirements.
