Key Responsibilities
- Assess residents’ health status through observation, physical assessment, and review of diagnostic information
- Administer medications and treatments in accordance with physician orders
- Participate in care planning and collaborate with the interdisciplinary team
- Monitor, document, and communicate changes in resident condition to physicians and nursing leadership
- Supervise and support nursing assistants and other care staff
- Educate residents and families on health conditions and care plans
Qualifications
- Current licensure as a Licensed Practical Nurse (LPN)
- Active Pennsylvania license (or eligibility)
- Strong clinical, communication, and organizational skills
- Experience in long-term care or skilled nursing preferred, but not required
- Caregiver with a compassionate, patient-centered approach to care
