Licensed Practical Nurse

About the role

FCHC is a Federally Qualified Health Center (FQHC) offering outpatient nursing positions in East Hartford, CT. Staff LPNs manage urgent and emergent patient calls, perform nursing functions, and assist with various healthcare tasks.

Responsibilities

• Manage urgent and emergent patient calls across all disciplines
• Perform licensed professional nursing functions to assess patient needs and severity
• Facilitate patient medication refills and manage provider schedules
• Counsel patients and perform related tasks as needed
• Provide direct and individualized nursing care based on the nursing process
• Maintain accurate medication inventory listings and oversee the CT Vaccine Program
• Document in the electronic health record, including assessments, vital signs, specimen collections, interventions, evaluations, and responses
• Manage prescriptions and medication refills for providers
• Perform wound care and adhere to infection control practices
• Document meaningful use data, including depression screening, sexual history, alcohol and substance abuse history, and assessment
• Referrals appropriate patients to specialty providers based on assessments
• Work with medical assistants to ensure safe and effective patient care
• Monitor task lists and respond to overdue orders and critical referrals
• Adhere to patient safety standards and infection control guidelines
• Aid in the smooth operation of the care team and delivery of excellent service

Requirements

• Experience with an electronic health record system
• Vaccine Administration experience
• Excellent communication and interpersonal skills
• A positive attitude and willingness to be cross-trained in multiple specialty areas
• Valid LPN license in the state of Connecticut
• BLS Certification

Qualifications

• Minimum 2 years of relevant experience
• Current BLS certification

• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Knowledge of electronic health records and vaccine administration
• Ability to manage multiple tasks and prioritize effectively

Pay

The hourly pay range for this position is $27.00 - $33.00/hour. Pay is based on experience, certifications, and other factors.

Schedule

Day shift position; start time is 8:00 am; minimal rotation on Saturday shift.
