Key Responsibilities
- Provide direct patient care in clinic and via telephone, including assessments, medication administration, vaccinations, and patient education.
- Collaborate with RNs, providers, Medical Assistants, and interdisciplinary team members to develop and implement individualized care plans.
- Care for coordination of patient care, specialty referrals, anticoagulation management, and communication with community agencies and home health providers.
- Perform medication reconciliation, review laboratory results with patients as directed, and conduct follow-up assessments.
- Document accurately in the electronic medical record (EMR) and manage patient forms, messages, and care-related communications.
- Delegate and provide clinical oversight to Medical Assistants, supporting workflow efficiency and quality patient care.
- Respond to emergencies, maintain clinical supplies and medication inventory, and ensure compliance with safety and infection-control standards.
- Participate in quality improvement initiatives, team huddles, departmental meetings, and patient-centered care activities.
Requirements
- Current Connecticut Licensed Practical Nurse (LPN) license required.
- Graduate of an accredited nursing program.
- Minimum 2 years of nursing experience required; primary care or fast-paced ambulatory care experience preferred.
- Current BLS certification.
- Strong clinical assessment, critical thinking, and problem-solving skills.
- Excellent verbal, written, and electronic communication skills.
- Ability to work independently and collaboratively in a team-based environment.
- Proficient with EMR systems and computer applications.
