About the Role

As a Licensed Practical Nurse (LPN) at Holmes Lake Rehabilitation & Care Center, you will play a crucial role in delivering high-quality, compassionate care to our residents. You will work in a supportive and dynamic environment where your skills and dedication contribute significantly to the well-being of our residents.

Responsibilities

  • Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.
  • Monitor residents’ vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing.
  • Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident’s needs.
  • Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team.
  • Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.
  • Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.
  • Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care.

Requirements

  • Valid LPN/LVN license in Nebraska.
  • Graduation from an accredited LPN/LVN program.
  • Previous experience in a skilled nursing facility or similar healthcare setting preferred.
  • Current CPR/BLS certification required. Additional certifications may be required based on state regulations.
  • Excellent verbal and written communication skills.
  • Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and their families.
  • Ability to lift and move patients, perform physical assessments, and stand for extended periods.
