Job Summary
The Licensed Practical Nurse (LPN) oversees client care, medical records, health and safety, medication administration, and coordinates medical care for clients with psychiatric and health-related needs.
Key Responsibilities
- Provision of assistance to residents who are unable to self-administer medication.
- Supervision of self-administered medication when indicated.
- Knowledge of and Administration of injectable medications as prescribed.
- Participation in education of residents regarding their medications.
- Immediately reporting of medication errors to supervisor.
- Timely review and notification to providers of laboratory results.
- Clozaril REMS data entry.
- Infection control: Strict adherence to infection control guidelines.
- Timely reporting of all crises and unusual incidents to Director of Nursing and LTSR Director.
- Immediate reporting of psychiatric and medical crises to Psychiatrist and Associate Medical Director.
- Compliance with all internal and external requirements and regulations.
- Observance of Universal Precautions in administering medications.
- Provision of direct service to residents, including assistance with personal hygiene, bathing/showering, and all other tasks of daily living as needed.
- Leading activities with residents, including educational, social, and leisure programs for which residents’ input is elicited.
- Participation in community integration activities as needed.
- Regular and relevant documentation in clinical chart and utilizes other designated forms.
- Attendance at all meetings and training sessions as assigned.
- Reporting of all non-emergent shift and shift personnel problems to Director of Nursing and/or LTSR Director in a timely and accurate manner.
- In the event of a staff call out, works to secure appropriate coverage via existing team members or other contracted/approved temporary staff organizations.
Requirements
- Active LPN License
- At least two (2) years of mental health experience preferred
- Valid driver’s license required (must be able to move about COMHAR service areas)
- Physical examination including TB test and Hep B
- Act 31 Mandated Reporter Training
