Job Summary
Under the direction and supervision of the Clinic Practice Manager, the LPN administers medications, implements primary care orders, and evaluates participant complaints physically and/or by telephone/telehealth. The LPN also provides education to the participant and caregiver in regards to care plan, nursing treatments or medications. The LPN communicates participant’s condition to primary caregivers, physician, and interdisciplinary team.
Essential Duties and Responsibilities
- Performs basic nursing services in the PACE clinic within scope of practice.
- Participates in planning and executing interventions in accordance with the participants plan of care.
- Administers prescribed medications, vaccinations, and treatments in accordance with nursing standards and documents appropriately.
- Withdraws blood from participants as instructed.
- Maintains participants’ medical record and fulfills PACE organization charting and reporting requirements.
- Observes, records, and reports participant’s condition and reaction to drugs and treatments to physicians.
- Triage participants within scope of practice to determine priority of care.
- Educates participants and family/caregivers regarding medications and treatment instructions.
- Prepares equipment and aids physician during treatment, examination, and testing of participants.
- Aids in the maintenance of the exam rooms for necessary supplies and materials.
- Ensures timely flow of participants.
- Responds to and refers incoming telephone calls.
- Aids in the scheduling of specialist appointments and assisting in hospital admissions and calling report to unit, as assigned or requested.
- Follows all BoldAge Policies and Procedures and OSHA safety guidelines.
- Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
- Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal precautions.
- Maintains a safe working environment.
- Follows BoldAge Safety Policies and Procedures.
- Participates in and supports Quality Improvement initiatives.
- Participates in continuing education classes and any required staff and training meetings.
- Maintains professional affiliations and any required certifications.
Experience and Education
- Graduate of an accredited school of nursing.
- One year working as an LPN strongly preferred.
- BLS required (must have within 90 days of employment).
- IV Certification preferred.
- One year of experience working with a frail or elderly population preferred.
- If not present, training on working with a frail or elderly population will be provided upon hiring.
Pre-Employment Requirements
- Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
- Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
- Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
