Job Summary

JOB SUMMARY
Under the direction and supervision of the Clinic Practice Manager, administers medications, implements primary care orders, and evaluates participant complaints physically and/or by telephone/telehealth. Provides education to the participant, and caregiver in regards to care plan, nursing treatments or medications. Communicates participant’s condition to primary caregivers, physician and interdisciplinary team.

Essential Duties and Responsibilities

  • Performs basic nursing services in the PACE clinic within scope of practice.
  • Participates in planning and executing interventions in accordance with the participants plan of care.
  • Administers prescribed medications, vaccinations, and treatments in accordance with nursing standards and documents appropriately.
  • Withdraws blood from participants as instructed.
  • Maintains participants’ medical record and fulfills PACE organization charting and reporting requirements.
  • Observes, records, and reports participant’s condition and reaction to drugs and treatments to physicians.
  • Triage participants within scope of practice to determine priority of care.
  • Educates participants and family/caregivers regarding medications and treatment instructions.
  • Prepares equipment and aids physician during treatment, examination, and testing of participants.
  • Aids in daily appointments and emergencies.
  • Maintains timely flow of participants.
  • Responds to and refers incoming telephone calls.
  • Aids in the maintenance of the exam rooms for necessary supplies and materials.
  • Ensures cleanliness.
  • Maintains clinic supplies on the reorder supply list as appropriate.
  • Maintains participants’ medical record and fulfills PACE organization charting and reporting requirements.
  • Communicates in interdisciplinary team meetings, clinic meetings or any other meetings as assigned.
  • Assists in the scheduling of specialist appointments and assisting in hospital admissions and calling report to unit, as assigned or requested.
  • Follows all BoldAge Policies and Procedures and OSHA safety guidelines.
  • Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
  • Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal precautions.
  • Maintains safe working environment.
  • Fulfills other duties as required or requested.

Experience and Education

  • Graduate of an accredited school of nursing.
  • One year working as an LPN strongly preferred.
  • Current LPN/LVN license in good standing to practice in the required state (NJ: New Jersey State Board of Nursing).
  • BLS required (must have within 90 days of employment).
  • IV Certification preferred.
  • One year of experience working with a frail or elderly population preferred.
  • If not present, training on working with a frail or elderly population will be provided upon hiring.

Pre-Employment Requirements

  • Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
  • Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
  • Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
