Licensed Practical Nurse
About the role
The Licensed Practical Nurse (LPN) provides skilled and compassionate nursing care to hospice patients in their place of residence, including private homes, nursing homes, assisted living facilities, or other settings the patient calls home. Working under the direction and supervision of a Registered Nurse, the LPN delivers individualized care in accordance with the established plan of care to promote patient comfort, safety, and quality of life at the end of life.
Responsibilities
- Delivers individualized care in accordance with the established plan of care to promote patient comfort, safety, and quality of life at the end of life.
- Performs assigned nursing and personal care tasks.
- Makes rounds to monitor and report changes in patient condition.
- Provides education to patients and caregivers.
- Documents care accurately and timely within the electronic medical record (EMR).
Requirements
- Graduate of an accredited practical nurse or vocational nursing program.
- Current LPN licensure in good standing with the Kentucky Board of Nursing.
- Minimum of two (2) years of nursing experience; community health/hospice, medical-surgical, or long-term care experience preferred.
- Demonstrates compliance with accepted professional nursing standards and practices.
- Understands and supports the philosophy of hospice care and the needs of terminally ill patients.
Qualifications
- Licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.

Compensation is commensurate with experience.
Schedule
Primarily Mondays - Fridays, 8:00am until 5:00pm, with occasional on-call.
Location
Whitley, Laurel, Clay, Knox, Bell, and Harlan Counties.
