Licensed Practical Nurse

About the Role

The Licensed Practical Nurse partners with the Director of Nursing to oversee the care of individuals, ensuring best practices and high-quality support. This role fosters a "One Team" approach, promoting collaboration among medical, clinical, and operations teams to achieve the highest standard of care. Responsibilities include traveling to homes, providing medication management, medical treatment, education, and support to Direct Service Providers.

Responsibilities

• Daily follow-up to monitor all resident health concerns and regular in-person follow-up at assigned sites.
• Administration of medications and vaccinations as requested by healthcare providers.
• Complete nursing assessments, diabetic foot assessments, and AIMs.
• Coordinate care to ensure continuity, including follow-through with verbal and written orders, medication changes, monitoring for drug interactions and side effects, administering injections per physician order, hospital admission and discharge follow-up, and other scheduled appointments.
• Routinely review medication counts and orders in accordance with healthcare needs.
• Assist with coordination of resident psychiatric, medical, dental, and optical appointments as needed.
• Participate in interdisciplinary team meetings as an integral member of the care team.
• Proactively work with an interdisciplinary team to overcome barriers to care, such as treatment or medication refusal.
• Perform DMA and medical skills hands-on training when applicable.
• Participate in medical on-call rotation.
• Assist with development and revisions of medical policies and procedures.
• Assist in the destruction of controlled substances as outlined by DEA guidelines.
• Responsible for triaging medical concerns.
• Audit the use of EHR and EMAR programs.
• Educate staff and individuals served regarding infection prevention, physical health, medication management, and medical skills, including the use of medical equipment.
• Act as a healthcare liaison and respond to families and guardians regarding health-related questions or concerns.
• Provide input/data/summaries as part of the Infection Control Plan when needed.
• Participate in the influenza campaign and provide data where requested.

Requirements

• Valid LPN license in the state of licensure.
• 2 years’ previous experience preferred.
• Maintains own mandatory training and certification requirements.
• Must pass a criminal background check.
• Must possess a valid Driver’s License.
• Proficient in speaking, reading, and writing the English language.
• Approved by state, federal, and government entities to work within BSLS programs.
• Ability to use office equipment, including BSLS information technology systems.

Schedule

This is a full-time position, Monday through Friday, 8:00 AM to 5:00 PM.
