Requirements

  • Licensed Practical Nurse (LPN) with effective leadership & assessment skills in a long-term care setting
  • Must have knowledge of state and federal regulations
  • Must have, as a minimum, two (2) years of experience as a supervisor in a hospital, nursing care facility, or other related health care facility
  • Must have, as a minimum, six (6) months training experience in rehabilitative and restorative nursing practices
  • Experience with MDS
  • Proficient in PA CMIL
  • Licensed Practical Nurse Assessment Coordinator (LPNAC) must have Medicare billing knowledge and experience
  • Knowledge of PPS (PDPM)
  • Experienced in Insurance updates
  • Successfully complete a post offer, pre-employment criminal background check and physical / drug screen

Qualifications

Spring Creek Rehab and Nursing is seeking a full-time Licensed Practical Nurse Assessment Coordinator (LPNAC) to assist with leading their nursing team. The ideal candidate should have a compassionate and enthusiastic approach, at least two years of supervisory experience in a healthcare setting, and a minimum of six months of training experience in rehabilitative and restorative nursing practices. They must also have experience with MDS, be proficient in PA CMIL, and have Medicare billing knowledge and experience with PPS (PDPM).

Skills

The successful candidate will need to demonstrate strong leadership and assessment skills, as well as a thorough understanding of state and federal regulations. They should also be experienced in insurance updates and have a proven track record of success in a long-term care setting.

Pay
