Licensed Physical Therapist

About the role

A licensed Physical Therapist is sought for a skilled nursing and rehabilitation community in Littleton, CO, offering part-time and full-time roles at $45–$52/hr DOE.

Responsibilities

• Manage a structured caseload of residents
• Develop personalized PT plans focused on mobility, independence, and discharge outcomes
• Perform initial evaluations
• Implement therapeutic exercises
• Provide gait and balance training
• Assess and provide transfer and assistive device training
• Coordinate with interdisciplinary teams
• Educate families about patient care
• Document all activities in compliance with regulations

Requirements

• Active Colorado PT license
• Degree from an accredited physical therapy program
• Prior SNF or post-acute care experience preferred
• Strong communication and documentation skills
