About the Role

Reviews assessments at time of admission, determines clinical interventions, facilitates therapy, and provides education for patients and their families. Facilitates treatment planning and manages each case, including development of transition/discharge plans. Communicates effectively and respectfully with interdisciplinary teams and all other contacts.

Responsibilities

• Demonstrates clinical competence in the treatment planning process, including facilitating treatment planning and overseeing case management.
• Coordinates and facilitates the development of treatment plans by collaborating with patients, families, and the interdisciplinary team.
• Develops individualized plans based on age, social and cultural considerations, clinical diagnosis, level of functioning, and other relevant needs.
• Implements, reassesses, and adjusts treatment plans as necessary.
• Completes and updates interdisciplinary assessment tools for each patient weekly.
• Communicates frequently and effectively with the interdisciplinary team to ensure continuity and coordination of care.
• Ensures documentation is thorough, timely, legible, clinically appropriate, and meets regulatory requirements.
• Develops clinically relevant transition/discharge plans in collaboration with patients, families, and the interdisciplinary team.
• Conducts individual therapy, group therapy, family therapy, and psycho-education groups.
• Advances evidence-based practice by utilizing standards, guidelines, and pathways for care delivery.
• Promotes a culture of evidence-based practice through activities such as:
  • Utilizing evidence-based policies in clinical practice.
  • Attending and participating in learning opportunities about evidence-based practice.
  • Membership in hospital or unit-based practice/research councils.
  • Participation in evidence-based practice projects, literature reviews, or research studies.
  • Development of standards, pathways, or guidelines for patient care.
  • Presentations or research studies.

Requirements

• Master’s degree in psychology, social work, nursing, or a closely related field.
• One of the following current Minnesota licenses:
  • Licensed Independent Clinical Social Worker (LICSW),
  • Licensed Psychologist (LP),
  • Licensed Professional Clinical Counselor (LPCC), or
  • Licensed Marriage and Family Therapist (LMFT).
• Licensed Alcohol and Drug Counselor (LADC).
• Basic Life Support certification prior to the end of orientation.

Preferred Qualifications

• 2 years of supervised therapy experience in mental health/substance use disorders, including assessment, individual, group, and family therapy.

Pay

The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on factors such as FTE, skills, knowledge, relevant education, experience, and market conditions. Hiring at the maximum of the range is not typical. If eligible, a sign-on bonus may be offered based on the approved program at the time of the offer.
