Licensed Mental Health Professional – In-Person SNF Services (Psychologist, LMHC, LMFT, LPC)

We are seeking fully licensed mental health professionals to provide in-person psychotherapy and behavioral health services. Responsibilities
- Provide in-person psychotherapy and behavioral health services within Skilled Nursing Facilities and Assisted Living communities.
- Conduct diagnostic assessments and develop treatment plans.
- Treat depression, anxiety, adjustment disorders, grief, trauma, and other behavioral health concerns common among geriatric populations.
- Maintain timely and accurate clinical documentation.
- Collaborate with interdisciplinary care teams to improve patient outcomes.
- Build therapeutic relationships with residents and their families.
Requirements
- Active and unrestricted New York license as one of the following:
- Clinical Psychologist (PhD/PsyD)
- Licensed Clinical Social Worker (LCSW)
- Licensed Mental Health Counselor (LMHC)
- Licensed Marriage & Family Therapist (LMFT)
- Master’s degree or doctoral degree in an applicable discipline.
- Ability to provide compassionate, patient-centered care.
- Strong communication and documentation skills.
- Comfortable providing services in nursing home and long-term care settings.
Qualifications
Preferred:
- Experience working with geriatric populations.
- Experience in Skilled Nursing Facilities, rehabilitation centers, long-term care, or assisted living settings.
- Interest in interdisciplinary and integrated healthcare.
Important Licensing Requirement: Only fully licensed providers will be considered. Applicants with the following credentials are not eligible: Limited Permits, Associate Licenses (LMSW, MHC-LP), Provisionally Licensed Therapists, or those with supervised/pre-licensure status.
