Licensed Mental Health Professional – In-Person SNF Services (Psychologist, LMHC, LMFT)

Bring compassionate mental health care to older adults in Skilled Nursing Facilities (SNFs) and Assisted Living communities. About the role

• Provide in-person psychotherapy and behavioral health services within Skilled Nursing Facilities and Assisted Living communities.
• Full-time, part-time, and flexible scheduling options available.

Responsibilities

• Conduct diagnostic assessments and develop treatment plans.
• Treat depression, anxiety, adjustment disorders, grief, trauma, and other behavioral health concerns common among geriatric populations.
• Maintain timely and accurate clinical documentation.
• Collaborate with interdisciplinary care teams (physicians, nurses, rehabilitation staff) to improve patient outcomes.
• Build therapeutic relationships with residents and their families.

Requirements

• Active and unrestricted New York license as one of the following:
  • Clinical Psychologist (PhD/PsyD)
  • Licensed Mental Health Counselor (LMHC)
  • Licensed Marriage & Family Therapist (LMFT)
  • Licensed Clinical Social Worker (LCSW)
• Master’s degree or doctoral degree in an applicable discipline.
• Ability to provide compassionate, patient-centered care.
• Strong communication and documentation skills.
• Comfortable providing services in nursing home and long-term care settings.

Preferred Qualifications

• Experience working with geriatric populations.
• Experience in Skilled Nursing Facilities, rehabilitation centers, long-term care, or assisted living settings.
• Interest in interdisciplinary and integrated healthcare.
