About the role

The Licensed Mental Health Professional (LMHP) plays a crucial role in the Behavioral Health team at COFMC. They assist clients in resolving interpersonal conflicts and managing psychosocial stressors to improve their daily functioning.

Responsibilities

  • Conduct initial BH screenings and psychosocial assessments on all new clients under the supervision of the BH Director.
  • Prepare and update initial client treatment plans and conduct treatment reviews every 3-6 months, which are tailored to each client's unique situation, including measurable and achievable goals and objectives.
  • Complete client discharge summaries within 30 days of care termination.
  • Provide clinical case management services, individual therapy, and family therapy to clients meeting the BH Department's admission criteria.
  • Document services using an electronic health record (EHR) in accordance with Behavioral Health department practices, ensuring timely and accurate entries.
  • Aid in the processing of all referrals received and attend required meetings, trainings, and events as directed by the Director of Behavioral Health and COFMC Administration.

Qualifications

  • Master’s Degree from an accredited institution in Social Work (with a clinical specialty) or a related Human Services field.
  • Current license as a Licensed Clinical Social Worker, Licensed Professional Counselor, or Licensed Behavioral Professional.
  • On-site supervision available for social workers seeking or eligible for licensure.
  • Knowledge of current social and economic issues, family economics, human relations, community resources, and psychological terminology.
  • Ability to demonstrate knowledge through the COFMC Competency Assessment for Behavioral Healthcare Practitioners.
  • Understanding of emergency procedures, clinical policies, and infection control measures.
