About the Role

This is a full-time Licensed Master Social Worker position in a fast-paced office setting at a Magnet-recognized hospital in the Capital Region. The role offers strong orientation, generous tuition allowance, career development, and a Monday-Friday office-hour schedule.

Responsibilities

  • Provide advanced clinical social work services as part of an interdisciplinary team, supporting patients and families in coping with hospitalization, illness, diagnosis, treatment, and life situations, including emotional, mental, and substance abuse disorders.
  • Conduct thorough assessments, care planning, case management, and community resource coordination.
  • Use motivational interviewing, assessment, and multi-modality psychotherapeutic interventions to positively affect patients' physical, cognitive, and emotional functioning.
  • Provide specialized skills and therapeutic education for patients and their support systems.
  • Identify barriers impacting optimal patient functioning and ensure patients receive appropriate resources in the most suitable setting.
  • Conduct comprehensive psychosocial assessments.
  • Collaborate with the interdisciplinary care team, leading complex cases, providing education, mentoring, modeling, and resources.
  • Act as a patient advocate, promoting autonomy and championing patients' rights to self-determination.
  • Develop, implement, evaluate, and revise care plans for hospital and post-hospitalization, including referrals to other disciplines and community organizations.
  • Identify areas for dynamic intervention and utilize brief multi-modality psychotherapeutic interventions.
  • Participate in Patient Rounds, Unit Care Conferences, Complex Care Committee, and other patient-centered teams.
  • Serve as an advanced clinical consultant and educator to patient-focused care teams.
  • Attend and document participation in interdisciplinary teams, staff meetings, patient care meetings, and Quality Assurance/Improvement activities.
  • Actively participate in hospital, organizational, and community-based initiatives to enhance patient care.

Requirements

  • Master's degree in Social Work is required.
  • NYS LMSW or LCSW licensure is required.
  • Minimum of 1 year of experience in a medical, psychiatric, or healthcare setting is preferred.

Pay

Pay Range: $30.00 - $43.08 per hour, based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may vary based on location.
