Responsibilities
- Providing crisis intervention, service linkages, and discharge planning
- Developing treatment plans with individuals and treatment team; monitoring and evaluating progress; and modifying treatment plans as needed
- Determining discharge needs and appropriate community resources
- Establishing and maintaining relationships with community providers to ensure continuity of care
Requirements
- Licensed and currently registered by New York State as a Licensed Master Social Worker (eligible for NY HELPS program)
- One year of permanent competitive or 55‑b/55‑c service in a title eligible for transfer under Section 70.1 of the Civil Service Law (for internal State Service candidates)
- Maintain license/registration; failure may result in removal from position
- Must not be listed as an excluded individual/entity on Federal or State Medicaid/Medicare exclusion lists; appointment may be terminated if later listed
- May be required to become an enrolled Medicare provider, obtain an NPI, and participate to allow billing through Medicare/Medicaid; loss of billing ability may lead to termination
- Background check required
