About the role

Manhattan Psychiatric Center is recruiting for a Licensed Master Social Worker to provide professional social work services to individuals with mental illness as part of the Mobile Integration Team. This team delivers an array of mobile services and supports for adults, including on-site crisis assessment, family support, and respite in a recovery-oriented environment.

You will be part of a multidisciplinary team providing culturally competent and evidence-based treatment and support services.

Responsibilities

  • Provide crisis intervention, service linkages, and discharge planning.
  • Develop treatment plans with individuals and the treatment team; monitor and evaluate progress; and modify treatment plans as needed.
  • Determine discharge needs and identify appropriate community resources.
  • Establish and maintain relationships with community providers to ensure continuity of care.
  • Drive a state vehicle to deliver mobile services and supports (valid driver’s license required).

Requirements

  • Licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW).
  • Must maintain licensure and registration; failure to do so may result in removal from the position.
  • Must not be listed as an excluded individual or entity on any Federal or State Medicaid/Medicare exclusion lists (or excluded from any other Federal or Federally assisted program).
  • If appointed, may be required to become an enrolled Medicare provider, obtain a National Provider Identifier (NPI) number, and actively participate to allow billing through Medicare and Medicaid.
  • Valid driver’s license and ability to drive a state vehicle.

Candidates from outside or within State Service can be considered for hire under the Hiring for Emergency Limited Placement – Statewide (NY HELPS) program if they meet the LMSW licensure requirement.

Candidates within State Service may also be considered for transfer to a competitive class position if they have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.
