About the Role

We are looking for a compassionate Licensed Master Social Worker specializing in geriatric care to join our team. This full-time position is based in New York, NY, with a Monday to Friday schedule. The role involves working in the field and collaborating with a supportive team, requiring proficiency in general office software and electronic documentation systems.

Responsibilities

• Conduct regular home visits in NYC, Westchester, Long Island, and New Jersey.
• Attend weekly in-person team meetings at our office.
• Maintain flexibility to adjust schedules based on client needs.
• Work independently and as part of a team.

Requirements

• Possess a valid driver's license and reliable transportation.
• Strong organizational and time-management skills.
• Excellent interpersonal and communication skills.

Qualifications

• LMSW or LCSW required.
• Experience in working with older adults and care management.
• Knowledge of Medicare, Medicaid, Social Security, and community services.
• Proficiency with general office software and electronic documentation systems.

Pay

$70,000-$75,000 Per Year

Full Time - Mon-Fri
