Responsibilities
- Meet with all patients on caseload a minimum of once per week, unless clinically inadvisable (e.g., pt too disorganized to tolerate), in which case meetings should be adjusted to accommodate pt's needs.
- Complete screening, assessments, notes, care plans, education within time frames in the EMR.
- Execute discharge plans, including referrals for home care and Skilled Nursing Facilities.
Requirements
- 02 years of experience in a hospital facility.
