About the role

Assess participants' social, emotional, economic, and environmental needs, and develop appropriate intervention plans; home visits may be required.
Conduct interviews, prioritize needs, and accurately document case activities and progress.
Collaborate as a team-oriented department with managers, other departments, agencies, and caregivers to ensure alignment with treatment plans and continuity of care.
Connect participants and caregivers with community resources, supporting discharge planning and ongoing post-care needs.

Requirements

  • Licensure / Certification / Registration: Preferred Credential(s)
  • Drivers License.
  • Masters Level Social Worker.
  • Education: Master's degree in Social Work required.
  • Work Experience: 3 years of experience preferred.
