Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans.
- Provide skilled therapeutic interventions to improve mobility, strength, balance, and function.
- Educate patients and caregivers on exercises, safety, and home programs.
- Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate care.
- Complete accurate and timely clinical documentation.
- Monitor patient progress and adjust treatment plans as appropriate.
Qualifications
- Current Colorado Physical Therapist (PT) License in good standing.
- CPR certification.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage patient visits effectively.
- Home Health experience is preferred but not required.
- Reliable transportation and a valid driver's license.
