About the role
Reporting to the Clinical Manager, you will plan and administer prescribed physical therapy treatment for patients in their homes, aiming to attain the highest level of physical function. You will assess and document patients' physical strengths, develop and implement rehabilitation programs, and collaborate with physicians, PTAs, Home Health Aides, and care teams to support patient goals.
Responsibilities
- Plan and administer prescribed physical therapy treatment to help patients attain the highest level of physical function.
- Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement a rehabilitation program consistent with the physician's Plan of Treatment and patient team goals.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
- Document patient observations, interventions, and evaluations promptly and thoroughly.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
Requirements
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid state driver's license and maintenance of personal vehicle liability insurance (minimum $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage or state-mandated limits).
Qualifications
- Required: Experience as a Physical Therapist in a healthcare setting.
- Preferred: One year of experience as a Physical Therapist.

Pay
- Per Visit/Unit Rate: $76.00 - $106.00.
- Annual pay range (full-time, 40 hours per week): $111,600 - $153,600, based on geographic location, skills, knowledge, experience, education, and certifications.
Schedule
- Scheduled weekly hours: 40.
Additional Information
- This role is patient-facing and requires Tuberculosis (TB) screening.
- This role is part of Humana's driver safety program.
