Home Health Physical Therapist

About the role

Home Health Physical Therapist responsible for delivering personalized in-home physical therapy to improve patients' strength, mobility, and independence.

Responsibilities

• Plan, administer, and document prescribed physical therapy treatments to attain optimal physical function.
• Assess patient strengths, develop and implement rehabilitation programs aligned with physician plans and patient goals.
• Supervise and provide clear instructions to PTAs and Home Health Aides, overseeing their performance and documentation.
• Maintain thorough documentation of patient observations, interventions, evaluations, and progress reports.
• Communicate patient progress to physicians, care teams, patients, and families, submitting all required reports and summaries.

Qualifications & Skills

• Degree from an accredited Physical Therapy Program.
• Unrestricted Physical Therapy license and current CPR certification.
• Experience working collaboratively within healthcare teams.
• Preferred: One year of clinical experience as a Physical Therapist.

Additional Information

Role is patient-facing, requiring TB screening and a valid driver’s license with personal vehicle insurance. Compensation includes per visit and annual salary ranges, with benefits supporting whole-person well-being.
