Licensed Clinician - (LISW, LMHC, LMFT)- Cedar Rapids, IA

About the role

MPAC Healthcare is looking for Licensed Clinicians (LMHC, LMFT, LISW) who are excited to set a new bar for the quality of care provided to seniors. We are team-oriented, reliable providers and professionals who are driven with integrity providing high-quality care in the senior care space.

Responsibilities

• Function as an active, lead member on an interdisciplinary team of physicians and nurse practitioners addressing residents’ emotional, social, and psychological needs.
• Provide initial assessments and utilize psychotherapy to address mental health issues and offer ongoing support.
• Responsible for all documentation requirements and keeping up-to-date on Medicare Benefit and commercial insurance plans.
• Work on-site as a clinical provider with residents and their families.
• Manage your own caseload, schedule, and salary within a 40-hour work week.
• Collaborate with interdisciplinary teams, including other MPAC providers, to ensure patients receive the best care possible.
• Foster open communication with the facility team and your Regional Clinical Manager to ensure the success of clinical programs.
• Take initiative and effectively communicate in multiple settings with patients, families, nursing staff, facility social services, and other providers.
• Maintain high level of organization to manage patient needs, document effectively, and ensure mental health needs of residents are met.

Requirements

• Master’s degree and a valid LISW/LMFT/LMHC license in the state for which you are applying.
• Two years of counseling and therapy experience.
• Ability to work as a member of a team with a great attitude.
• Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches.
• Experience collaborating with interdisciplinary teams.
• Ability to foster open communication with facility teams and Regional Clinical Manager.
• Ability to take initiative and communicate effectively with multiple parties (patients, families, nursing staff, facility social services, other MPAC providers).
• High level of organization to manage patient needs and document effectively.
• Comfort with the ever-changing environment of a skilled nursing facility.
• Knowledge of Medicare Benefit and commercial insurance plans.
• Experience in the healthcare setting.
• Passion for working with older adults.
• Exceptional organizational skills.
• Experience in the nursing home space is a plus.

Schedule

Ability to work within a 40-hour work week with the majority of time clinically focused and the remainder focused on patient documentation and facility correspondence.
