Position Summary

The Palliative Care Social Worker is an integral member of the Palliative Care Interdisciplinary Team which delivers patient- and family-centered care to individuals of all ages living with serious illness. The goal of palliative care is to maximize the quality of life and align the individual's medical care with what matters most to them.

Key responsibilities

  • Evaluate the psychosocial needs of patients and families
  • Provide targeted counseling including anticipatory grief support
  • Facilitate family meetings and goal-setting discussions
  • Make referrals to community resources
  • Engage in effective interdisciplinary team collaboration

Required Minimum Knowledge, Skills, And Abilities (KSAs)

  • Education: Master's Degree in Social Work/Clinical Counseling from an institution accredited by the Council on Social Work/Counselor Education required.
  • Certifications: Current applicable state(s) license as an LCSW (Licensed Clinical Social Worker) or LMSW-CC (Licensed Master Social Worker, Conditional Clinical) required.
  • Experience: Two years of relevant experience in a health or mental health setting preferred. Demonstrates competency in all essential functions of a Social Worker I role.
