About the role

The Licensed Clinical Social Worker participates as an integral part of a multidisciplinary team to establish comprehensive and integrated plans for each patient. Involvement entails both delivery of clinical services and referrals to appropriate community resources and other professionals. Assists patients and their families in meeting psychosocial needs which predispose illness or interfere with achieving an optimal level of care. Assesses the social, psychological, cultural, environmental and financial situations, as well as disposition needs, for each assigned patient.

Responsibilities

  • Participates in multidisciplinary rounds and rounds of care planning
  • Assesses the social, psychological, cultural, environmental and financial situations, as well as disposition needs, for each assigned patient
  • Delivers clinical services to patients and their families
  • Refers patients and their families to appropriate community resources and other professionals
  • Collaborates with other healthcare providers to develop and implement comprehensive care plans
  • Provides support groups and other psychosocial interventions
  • Assists patients and their families in navigating the healthcare system and addressing financial concerns
  • Conducts assessments and evaluations as needed
  • Participates in quality improvement initiatives and research projects

Requirements

  • Master's degree in social work
  • Clinical Social Work License valid in state of practice (LCSW)
  • BLS certification (Required within 90 days)
  • 2 years' social work experience
  • Prior palliative or end-of-life care work experience (Strongly preferred)

Qualifications

  • Must be able to pass a criminal background check
  • Must be able to pass a drug screen
  • Must be able to pass a physical examination

  • Strong organizational and time management skills
  • Knowledge of community resources and referral networks
  • Ability to handle sensitive and confidential information
