About the role

The Licensed Clinical Social Worker (LCSW) is a key member of the Health Promotion & Disease Prevention Team within the Department of Social Work & Community Health (SWaCH) at Rush University Medical Center. This role offers clinical support at the Sankofa Village Wellness Center (Sankofa) and is responsible for the coordination, implementation, and oversight of Rush Generations evidence-based programming for individuals with chronic illnesses and older adults.

Responsibilities

  • Works directly with patients and their families to address unmet needs and provide the necessary social work services them achieve their care goals.
  • Carr[ies out comprehensive social work assessments by phone or in-person to develop a plan of care for the patient and/or family.
  • Connects patients and their families to relevant resources, both at Rush and in the community, to address their needs.
  • Provides care management and/or support programming for aforementioned population as outlined by Manager.
  • Documents all client interactions in either the Epic, LCM software and/or other spaces as indicated by Manager.
  • Maintains relationships with referring Rush clinics through consistent and timely communication.
  • Promotes Rush University System for Health clinical services by referring clients to relevant Rush clinics.
  • Represents the department at meetings, conferences or other public engagements on an as-needed basis.

Requirements

Required Job Qualifications:

  • Master’s Degree in Social Work.
  • Licensed Social Worker (LCSW) in State of Illinois.
  • Two years’ experience in healthcare, care management, mental health and/or social services.

Preferred Job Qualifications

  • Experience with relevant population(s).
  • Professional qualities such as initiative, self-reflection, commitment, responsiveness and adaptability.
  • Knowledge of care management, community-based interventions, assessment, counseling and/or motivational interviewing.
  • Computer literate (Microsoft Office and others).
