Location: Chicago, Illinois – Rush University Medical Center, Social Work & Community Health
About the role
The Licensed Clinical Social Worker is a key member of the Rush University System for Health community. This position provides social work services from a variety of referral sources and connects patients and community members to relevant resources both internal to Rush and in the community. This role supports relevant programming for the population served and may represent the department in meetings and presentations. The position exemplifies the Rush mission, vision, and values and acts in accordance with Rush policies and procedures.
Responsibilities
- Works directly with patients and their families to address unmet needs and provide necessary social work services to help them achieve their care goals.
- Conducts comprehensive social work assessments by phone or in-person to develop a plan of care for the patient and/or family.
- Connects patients and their families to relevant resources, both at Rush and in the community, to address their needs.
- Provides care management and/or support programming for the population as outlined by the Manager.
- Documents all client interactions in Epic, LCM software, and/or other designated systems.
- Maintains relationships with referring Rush clinics through consistent and timely communication.
- Promotes Rush University System for Health clinical services by referring clients to relevant Rush clinics.
- Represents the department at meetings, conferences, or other public engagements as needed.
Requirements
- Master’s Degree in Social Work.
- Licensed Clinical Social Worker (LCSW) in the State of Illinois.
- Two years’ experience in healthcare, care management, mental health, and/or social services.
Preferred Qualifications
- Experience with relevant population(s).
- Professional qualities such as initiative, self-reflection, commitment, responsiveness, and adaptability.
- Knowledge of care management, community-based interventions, assessment, counseling, and/or motivational interviewing.
- Computer literacy (Microsoft Office and other relevant software).
Pay
$29.36 - $47.79 per hour. Rush salaries are determined by factors including education, job-related experience, skills, internal equity, and industry-specific market data. Offers may vary depending on individual circumstances.
Schedule
- Full Time (Total FTE between 0.9 and 1.0).
- Shift 1: 8-hour work schedule (9:00 AM – 5:00 PM).
