We offer PRN positions for experienced Licensed Clinical Social Workers who can provide patient-centered care in patients' homes and communities.

Responsibilities

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification
• Communicate with families, professionals, and co-workers to support patient care

Qualifications

• Completion of a Master's degree in Social Work and state licensure as an LCSW
• Excellent planning, organization, writing, and time management skills
• Ability to communicate effectively with families, professionals, and co-workers
