About the role

Collage Home and Community Rehabilitation Partners is a dynamic company dedicated to providing exceptional neurological rehabilitation. Located in Pasadena, CA, we offer PRN positions for experienced Licensed Clinical Social Workers who can provide patient-centered care in the comfort of patients' homes and communities.

Responsibilities

Include, but are not limited to:

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification
• Communicate effectively with families, professionals, and co-workers

Qualifications

• Completion of a Master's degree in Social Work and state licensure as an LCSW
• Excellent planning, organization, writing, and time management skills
