About the Role
Strive Health partners with skilled nursing facilities to deliver high-quality behavioral health care to residents. Our integrated model combines psychiatric nurse practitioners and licensed clinical social workers to provide a full spectrum of mental health services at no cost to the facility. We handle scheduling, credentialing, and billing so our clinicians can focus on what they do best: patient care.
Responsibilities
- Conduct initial diagnostic assessments and develop individualized treatment plans.
- Provide psychotherapy services tailored to resident needs.
- Deliver individual, group, and family counseling focused on depression, anxiety, grief, adjustment, and behavioral health issues.
- Document sessions in compliance with Medicare and facility requirements.
- Participate in care plan meetings and interdisciplinary team discussions.
- Support residents and families in coping with illness, placement, and end-of-life issues.
- Collaborate with Strive psychiatric nurse practitioners and facility staff to ensure coordinated care.
- Educate facility staff on recognizing psychosocial needs and appropriate referral processes.
Qualifications
- Master's in Social Work (MSW) with active LCSW license in the state of Wisconsin.
- Enrolled or eligible for enrollment as a Medicare Part B provider.
- Experience working with geriatric populations or in long-term care preferred.
- Strong interpersonal, documentation, and communication skills.
- Ability to work independently within a collaborative, team-based environment.
