About ComoCare ComoCare is a behavioral health provider built to support community health centers and the people they serve. We deliver in-person intensive outpatient therapy (IOP) to communities that have historically been underserved in behavioral health care. Role Description As a Licensed Clinical Social Worker at ComoCare, you’ll play a central role in delivering high-quality, evidence-based care to patients participating in our IOP. You’ll lead group therapy sessions, build strong therapeutic relationships, and collaborate with our care team to ensure each patient’s treatment plan supports lasting improvement and recovery. As a ComoCare therapist, you will support refining the clinical curriculum as the program evolves and expands. Responsibilities Include, But Not Limited To, The Following Provide high-quality, evidence-based individual and group therapy within ComoCare’s intensive outpatient program (IOP) modelDeliver compassionate, patient-centered care that meets each individual’s unique behavioral health needsSupport patients through the full course of care — from intake to completion — ensuring strong engagement and retentionMonitor progress, outcomes, and adherence to care plans while maintaining accurate clinical documentationCollaborate with other members of the care team to coordinate holistic support for each patientContribute to the ongoing refinement of ComoCare’s clinical curriculum and care delivery model based on real-world insightsCommunicate and partner with the patient’s FQHC care team, including but not limited to the patient’s primary care provider and existing behavioral health provider, to ensure each patient receives the appropriate care and resourcesTrack and help drive key performance indicators (KPIs) including clinical quality, retention, completion, and patient satisfaction (NPS) Reporting: This role will report to the Clinical Director Required Qualifications Licensed clinical social worker (LCW)4–5 years of experience providing behavioral health services in outpatient or community-based settingsExperience facilitating both group sessions and one-on-one counselingExperience collaborating with interdisciplinary care teams for behavioral health patients, including primary careDemonstrated ability to build rapport, establish trust, and deliver trauma-informed careSelf-motivated with the ability to work independently and in a team environmentHigh empathy quotient with a strong commitment to patient-centered careExcellent communication, documentation, and collaboration skills Preferred Qualifications Experience working within community health centers or similar settingsBackground in intensive outpatient (IOP) or partial hospitalization (PHP) programsFamiliarity with evidence-based treatment modalities such as CBT, DBT, MI, or ACTExperience using electronic health records (EHR) systemsBilingual (English and Spanish)Excellent organizational skills and attention to detail with a high degree of accuracy Highlights Of Our Comprehensive Benefits Package Health, Dental, Vision insurance401(k) with Company matchCompetitive payPaid vacation, holidays, and sick leaveContinuing education and licensure reimbursement You will collaborate with like-minded health care professionals who, like you, understand the importance and value of ComoCare’s high-quality, behavioral health care.
