Licensed Clinical Social Worker - Health Centers - Full-Time

This position is a team-based role at Dodson Avenue Community Health Center, integrating behavioral health services with primary care. The Social Worker provides counseling to behavioral health patients, supports self-management and behavior change, and coordinates care for the patient population.

Responsibilities

  • Provide direct patient care through brief and comprehensive assessments, development and implementation of treatment plans.
  • Conduct individual, family, and group therapy as needed.
  • Collaborate with psychiatrists, behavioral health coordinators, and primary care teams to integrate behavioral health services with primary care.
  • Perform brief psychiatric assessments of primary care patients and provide feedback to primary care practitioners regarding follow-up care.
  • Deliver targeted psychotherapy to promote behavioral change in primary care patients.
  • Assess, plan, coordinate, monitor, and evaluate depression screens and follow-up care to ensure quality outcomes.
  • Observe patient/family behavior, assess needs, and create treatment strategies.
  • Diagnose psychological, behavioral, and emotional disorders.
  • Consult with doctors, therapists, and medical professionals.
  • Participate in the practice’s quality improvement process and performance evaluation.
  • Assist psychiatrists in managing behavioral health patients to provide comprehensive care and support.
  • Communicate and document patient care clearly and completely.
  • Demonstrate knowledge of cultural differences and provide care tailored to age-specific, cultural, and social needs.
  • Educate and intervene to promote wellness, particularly in addiction-related cases.

Requirements

  • Master’s Degree in Social Work and state licensure (Clinical Social Worker licensed by the State of Tennessee).
  • Minimum of 2 years’ experience in counseling/therapy.
  • Strong clinical judgment and clear communication skills (written and verbal).
  • Self-directed with the ability to collaborate effectively across disciplines.
  • Ability to conduct both brief and comprehensive psychiatric assessments.
  • Knowledge of psychodynamic, cognitive, behavioral, developmental, and systems theory.
  • Experience conducting group, individual, and family therapy using accepted modalities.
  • Familiarity with DSM-IV-TR terminology and case formulation.
  • Compassion for patients and knowledge of addiction intervention.

Preferred Qualifications

  • Experience conducting support groups and psycho-educational lectures.
  • Familiarity with regulatory and insurance reimbursement issues.
  • Experience integrating behavioral health into primary care settings.
  • Bilingual (Spanish) proficiency.
