Responsibilities
- Provides assessments, treatments, and case management to patients and families.
- Is a resource person to healthcare professionals concerning psycho-social problems and community resources.
- Maintains performance improvement activities within the department.
- Collects and evaluates data for social services and discharge planning.
- Maintains an accurate and up-to-date knowledge of relevant regulations and standards regarding provisions of social services while implementing treatments and discharge planning.
- Completes all required assessments, such as Suicide Risk Assessment and Psycho-Social Assessment by due date.
- Affords individual and group therapy/family group therapy.
- Communicates and updates families when needed.
- Affords oversight of the intake of referrals and follows up with referral sources.
- Participates in the weekly treatment team meeting and ensures appropriate follow-up documentation.
- Ensures activity and socialization groups are occurring per schedule.
Requirements
- Master’s degree in social work or clinical nurse specialist for psychiatric mental health.
- Current licensure such as LCSW or LPC.
- Experience in psychiatric setting preferred.
Qualifications
- Knowledge of community resources for older persons.
- Knowledge of Medicare, Medicaid and insurance requirements for continued care.
- Familiarity of medical terminology and conditions as these impact treatment and discharge needs.
- Familiarity with medical team roles in hospital departments.
