About the Role
Provide in-person psychotherapy to the geriatric population in skilled nursing, sub-acute, and assisted living facilities.
Responsibilities
- Conduct individual and group therapy sessions, assessing, diagnosing, and treating emotional and behavioral health conditions.
- Develop and implement evidence-based treatment plans tailored to each patient.
- Collaborate with facility staff, interdisciplinary teams, and families to support patient care.
- Maintain accurate clinical documentation in compliance with regulations using a clinician-designed EHR system.
- Monitor patient progress and adjust treatment as needed.
Requirements
- Active, unrestricted LCSW license in the practicing state.
- Effective communication skills in English.
- Proficiency with electronic health records and technology.
- Strong organizational and documentation abilities.
- Ability to provide in-person services within facility environments.
- Physical mobility to move between patient rooms and areas.
- Full vaccination and booster may be required in some facilities.
Preferred Qualifications
- Clinical experience in geriatric, subacute, or long-term care settings.
- Experience treating depression, anxiety, and adjustment disorders; additional psych or addictions experience is a plus.
- Excellent time management and organizational skills.
