LICENSED CLINICAL SOCIAL WORKER - CASE MANAGEMENT

About the role

Northwest Specialty Hospital is seeking a team-oriented Licensed Clinical Social Worker to join our Case Management Team. This position provides initial evaluations, individual and group therapy, consultation services, and suicide risk analysis to our patient population.

Responsibilities

• Provides consultations to clients
• Conducts initial mental health evaluations resulting in treatment plans
• Assesses clients using standardized assessment/screening tools and measures
• Provides brief, solution-focused therapy
• Delivers group psychoeducation/treatment related to chronic medical conditions
• Offers information and referral services
• Performs care coordination
• Conducts crisis assessments/triage with appropriate interventions
• Provides other wellness-related services
• Documents services in accordance with requirements
• Performs individual psychotherapy with clients
• Handles documentation and scheduling in the electronic health record

Requirements

• Demonstrates eligibility for employment in the U.S.
• Master’s Degree in a related field
• Current LCSW licensure through Idaho Board of Social Work Examiners
• Current MSW pursuing a clinical license
• Demonstrated skills in verbal and written English communications for safe and effective patient care and to meet documentation standards
• Demonstrates initiative and discipline in time management and medical record review
• Effective interpersonal skills
• 3-4 years of relatable experience
• Current BLS Certification
