Responsibilities
- Provide mental health services to patients independently and collaboratively.
- Manage and evaluate patient care within a multidisciplinary care team.
- Collaborate with the team to meet the following patient/family/significant other needs:
- Response to psychosocial and psychodynamic needs.
- Coordination of community resources to ensure the transition to the least restrictive care environment.
- Seamless continuity of care process.
- Therapeutic intervention.
- Interface directly with patient/family/significant others and all other interdisciplinary members of the care team.
- Provide counseling and crisis intervention to patients and families with complex social services needs, including emotional/psychiatric issues, family violence or abuse, legal issues, or financial problems.
- Analyze the psychosocial and continuum of care needs of patients and their progress toward desired outcome goals.
- Function as an educator, healthcare resource, interdisciplinary consultant, and patient advocate.
- Apply strong organizational and prioritization skills, along with highly developed problem-solving and deductive reasoning skills, to ensure total quality patient care.
- Be dependable, self-directed, pleasant, adaptable, able to work with minimal supervisory guidance, flexible to meet patient/family and department/facility needs, and collaborate effectively with all team members.
Qualifications
- Master’s Degree in Social Work.
- Licensed Clinical Social Worker (LCSW).
- One year of experience as a social worker preferred.
