About the Role

The Social Worker-LCSW Outpatient provides mental health services to patients, working both independently and collaboratively to manage and evaluate patient care as part of a multidisciplinary team. This role addresses psychosocial and psychodynamic needs, coordinates community resources for smooth transitions to less restrictive care, ensures continuity of care, and delivers therapeutic interventions.

Responsibilities

• Interface directly with patients, families, and interdisciplinary care team members to provide counseling and crisis intervention for complex social services needs, including emotional/psychiatric issues, family violence or abuse, legal issues, or financial problems.
• Analyze psychosocial and continuum of care needs, track patient progress toward outcome goals, and serve as an educator, healthcare resource, interdisciplinary consultant, and patient advocate.
• Apply strong organizational, prioritization, problem-solving, and deductive reasoning skills to ensure high-quality patient care.
• Work dependably and self-directed with minimal supervision, adapt to changing needs, and collaborate effectively with all team members.

Requirements

• Master’s Degree in Social Work.
• Licensed Clinical Social Worker (LCSW).
• Must complete hospital and/or payer credentialing before starting the role.

Qualifications

Preferred: One year of experience as a social worker.
