Licensed Clinical Social Worker

About the role

The Per Diem Clinical Social Worker visits patient homes throughout Rhode Island to provide evaluations and information on resources that enable patients to remain at home safely while maintaining quality of life. As part of a supportive interdisciplinary team, the role requires professional communication in person, via telephone, and email, documentation of findings and outcomes using company software on an iPad, and includes mileage reimbursement for travel.

Responsibilities

• Visit patient homes throughout Rhode Island to conduct evaluations and provide resource information.
• Ensure patients can remain at home safely while preserving quality of life.
• Collaborate with an interdisciplinary team and communicate professionally in person, via telephone, and email.
• Document findings and outcomes using company software on an iPad.
• Receive mileage reimbursement for travel.

Qualifications

• Current Licensed Clinical Social Worker (LCSW) license in the state of Rhode Island.
